Users must sign this Indemnification Document prior to approval for use of the district facilities.
User shall procure and maintain in full force and effect during the term of this Agreement, the type(s) and minimum limits of insurance specified here.
This document is an example of the insurance information required for use of facilities at all schools within the School District of Philadelphia.
